Diversity Wellness provides corporate wellness programs that educate and motivate employees to make healthy changes in their daily life that will become permanent lifestyle changes. 

 

Our programming includes Taking Care of My Health, a comprehensive health promotion program in English and Spanish, as well as wellness programs customized for other specific minority groups. Our culturally and linguistically competent health promotion messages have an impact on awareness, values, beliefs and attitudes  - which  positively impact health behavior.

Healthy Change Programs

 

The Taking Care of My Health program content is delivered through 8-week Healthy Change Programs that address the prevalent health issues of each client's employee population.  At the beginning of the program, each participant completes either the on-line or paper based Personal Health Analysis.  Participants then receive a Personal Wellness Report  which includes  lifestyle improvement recommendations and suggestions to start one or more Healthy Change Programs.

 

 

 

The Healthy Change Programs are organized into focus areas critical to maintaining or improving health:

  Eat Well
  Be Active
  Stop Smoking
  Be Sun Safe
 

Stay Healthy

 

 

 

Each  8-week Healthy Change Program  is a personalized, interactive program that combines internet-based tools and information, telephonic coaching and printed materials with peer health educators to have employees and their families target lifestyle behaviors that are associated with diseases and occupational health.  

 

The web-based system, www.takingcareofmyhealth.com  , allows the employee to track his or her progress, record diet intake and activity levels, set weekly goals, create an action plan, and obtain personalized feedback. Each 8 week program also includes a standardized curriculum administered at the worksite by the promotora in a classroom setting.  Language-specific telephonic coaching further supports continued behavior modification. 

 

Don?t Stress  (No se estrese)

The Don't Stress program helps individuals learn the fundamental principles of stress management and wellness--then put those principles to work in their own lives.  During the eight week program, participants are given tips and suggestions on how to identify, minimize, and manage the daily stressors in their lives. Both on-line or on-site, the step-by-step program provides the following:

 

  • Down-to-earth examples and challenging worksheets
  • Assessments and inventories
  • Creative coping alternatives
  • Step-by-step action planning guides

     

  • Employees who complete this program learn how to better manage stress by developing skills to help be a better worker, parent or partner.

     

     

     

    Eat Well (Coma sanamente)

    Eat Well is comprised of 4 Healthy Change eight week programs. All participants are asked to complete the MealMatchTM Questionnarie before they begin a program and are tracked weekly against goals they set for themselves. Participants can plan or record their daily meals, and receive personalized feedback.  Each program teaches the skills needed in meal planning, shopping, food preparation, label reading and nutritional value in food to make healthier food choices.

     

    • My Healthy Diet Program provides a daily recipe, tips on healthy eating habits, easy to prepare meals and suggestions for small changes your employees can make that can have a big impact on how they feel.
    • The Healthy Change Weight Loss Program is designed to help participants make eating habit changes that will result in gradual weight reduction and eventually become healthy diet habits. We offer sensible weight loss strategies such as behavor modification, exercise, and portion control.
    • The Healthy Change Diabetic Program encourages gradual changes in eating habits and exercise for the diabetic that will result in better management of their diabetes.
    • The Healthy Heart Program is designed to help participants make healthy food choices using the DASH diet to help them reduce the risk of the three major factors for heart attack  and stroke ? high blood pressure, high blood cholesterol and excess body weight.

     

    Be Active (Póngase en marcha)

    The Be Active Program includes both the Fitness Fundamentals Program and the Walking Program, which emphasize the benefits of regular exercise. Employees are taught how to design, implement, and evaluate a personalized exercise program, with an emphasis on fitness fundamentals, injury prevention, and avoiding monotony in an exercise routine. During the eight week programs, participants are given tips on how to be more active, and are encouraged to gradually increase their activity levels. Each employee can track their daily activity, set goals for themselves, and receive personalized feedback.  

     

    Stop Smoking  (Deje de fumar)

    This eight week program offers a proven method to stop smoking through behavior change techniques with  a focus on relapse prevention and maintenance through continual reinforcement . This program may also include telephonic coaching and on-site education to target tobacco users in different stages of quitting. Participants are given the tips, tools and information they need to stop smoking and then maintain a tobacco free lifestyle. Program topics include preventing weight gain, the benefits of regular physical activity, coping with difficult situations, and communication strategies.

    Be Sun Safe  (Protéjase del sol)

    The Be Sun Safe Program encourages employees to work and play safely in the sun.  During this eight week program participants are instructed on how to prevent skin cancer,  how to perform a skin self-examination, and the danger signs of melanoma.  This program also includes a bilingual "Sun Safety Kit for Outdoor Based Workers" that complies with OSHA guidelines to educate outdoor-based employees about how to reduce the risk of skin damage by ultra violet rays. 

    Stay Healthy  (Manténgase saludable)

    Stay Healthy provides tips, tools and information to help employees protect their health, and the health of their family. Employees learn how to become an active participant in improving their own health by learning the appropriate screenings, immunizations, and health information recommended by age and gender.  The program includes a personalized health calendar, and the ability to use any of the site's tools and trackers.

     

    Once a participant enrolls in a program, they will receive weekly reinforcing communications which provide motivation and encourages continued program participation. At the end of a program, the participant can retake the same program, enroll in a different program, or switch to the Stay Healthy Maintenance Program which allows them to continue using the tools and trackers at their own pace.
